Description:Adds support for starting playback of MoH to arbitrary participants in a conference, via AMI and CLI.

IMPORTANT: This patch builds upon behavioural corrections in ASTERISK-19562, which must be applied first.

IMPORTANT: While implementing this, I discovered that, with ConfBridge as it is now, participants listening to MoH because marked users have not joined or because they are alone are, in many cases, not properly muted, which means they may be heard by others or recorded if the conference is monitored. I have written another patch that addresses that issue, but it builds upon the changes made in this one. This patch is available under ASTERISK-19726

Basically, same comments here as in ASTERISK-19726. Separating out the patch from any other patch dependencies so that it applies cleanly to trunk and fixes the issue on its own will make this much easier to get through. MOH should not imply mute by default, but if you are adding moh_implies_mute option that you mentioned ASTERISK-19726, then that would be fine. If you want to hold off on this until the other patch gets approved, that would work. I promise to work with you to get the fixes reviewed and approved ASAP.

I started working on an individual version of this patch, but decided to hold off until we got through the other issue and that it had everything that would make both of us happy.
