Description:{{chan_skinny}} currently performs its own transfers. It should stop doing that and use the Bridging Framework.

First cut at refactoring skinny to use ast_bridge_transfer*.

May merge the skinny_transfer_blind and _attended functions into a single function, but would like to see attended transfer in action first.

Based on the assumption that the core attended_transfer will cleanup the skinny side of the bridged channels on succesful transfer.

Tested blind transfers which work as expected.

Revised skinny transfer as a result of skinny changes.

Blind transfers work. Attended and blond transfers tested and working when using rev3 of the attended transfer patch (https://reviewboard.asterisk.org/r/2511/).
